This page was exported from Caledon Citizen [ https://caledoncitizen.com ] Export date:Fri Apr 19 3:06:21 2024 / +0000 GMT ___________________________________________________ Title: Hills of Headwaters Collaborative announces new Transformation Lead ---------------------------------------------------  By ALYSSA PARKHILL Local Journalism Initiative Reporter The Dufferin-Caledon Ontario Health Team has introduced a Transformation Lead to the Hills of Headwaters Collaborative Ontario Health Team.  Established in 2019, the Hills of Headwaters Collaborative was one of the first of its fellow 24 Ontario Health Teams across Ontario dedicated to enhancing the delivery of health care through new models. Ontario currently has 42 health teams covering 86 per cent of the population.  A Transformation Lead creates and implements “organizational framework” in order to improve the treatment and care for patients and providers with models that are said to be more patient-centered and efficient.  Alexander Lim is excited to begin this position within the Hills of Headwaters Collaborative Ontario Health Team. Lim has over a decade of regional, provincial and national levels of experience within the health sector. With a Masters of Public Health from the University of Waterloo, along with a Masters of Law from Osgoode Hall Law School, the team saw Lim as the perfect fit.  “I am very delighted to be joining the Hills of Headwaters Collaborative Ontario Health Team and look forward to leading the OHT as we embark on this next phase of planning and implementation,” said Lim in a statement. “My first priority will be meeting as many members of the collaborative as I can to get a full picture of the community and how I can serve it best.” The Hills of Headwaters Collaborative is a group of health care providers and organizations who have come together to contribute to the overall wellbeing of the Dufferin-Caledon community. The group is consistently working at developing new and efficient ways of improving the health care system within the region.  “While it certainly is an unusual time to be joining a new team, we look forward to supporting Alex in his transition.
